Every now and then, I come across a patient who had been to so many doctors, taken a cocktail of medicines, and have given himself or herself a bucketful of differential diagnosis from acceptable to absurb (thanks to google). These things would either cut my differentials to a significant length or complicate a rather simple case all together, leaving me with a thick, tangled up case to work on. This things boil down to one thing, a rotting, profit oriented medical health system, thay demoralizes doctors' approach to an otherwise simple case. HMOs have capitalized so much on the health care system in a masquerade of security that it has boggled up medical judgement and care. I have nothing against medical insurance but the concept of influencing doctors to decide on a treatment plan not based on a patient's best care guide, but rather, on the HMO's best acceptable and profitable approach, is down right ugly. The profession had been downgraded in to a marketting instrument. Gone were the days when doctors were independent intrapraneurs, uncumbered by the influence of monsters such as the HMOs. Gone were the days when the clinical eye was the best medical diagnostic modality, and the doctors' opinion would always be based on the patient's circumstances and best care.
